Job Description - Client Care Coordinator

Description

 The Client Care Coordinator is a vital member of the Isaiah House team, supporting the clinical department by coordinating telehealth services and managing client appointments. This role ensures that clients receive the appropriate amount of care by maintaining accurate provider schedules and supporting the delivery of timely, HIPAA-compliant clinical services. 

Requirements

 

Qualifications

  • High School Diploma or GED required
     
  • Working knowledge of basic computer operations and common software programs
     
  • Strong communication and interpersonal skills
     
  • Self-motivated with the ability to stay on task and meet deadlines
     
  • Problem-solving and critical thinking skills
     
  • Ability to enforce program guidelines and support compliance
     
  • Highly organized with strong attention to detail

 

Key Responsibilities

  • Receive and schedule clinical telehealth requests from clients and providers
     
  • Coordinate appointment dates/times across providers to ensure timely service delivery
     
  • Maintain accurate, up-to-date calendars for clinical staff
     
  • Adhere to HIPAA laws and maintain strict confidentiality of client information
     
  • Follow all program policies, procedures, and documentation standards
     
  • Interact professionally with staff and clients across multiple departments
     
  • Manage multiple priorities and tasks efficiently in a fast-paced environment
     
  • Collaborate with multidisciplinary teams to support holistic client care
     
  • Support recovery programming related to physical health as needed
     
  • Uphold ethical standards and professional conduct
     
  • Track therapist case loads and assign therapists to the new clients intake
