Georgina Home Care is a licensed non-medical home care agency serving clients across Connecticut. We are committed to delivering compassionate, high-quality care that promotes dignity, independence, and safety for every client. We are seeking a dedicated and experienced Client Care Supervisor to oversee day-to-day client care operations, ensure compliance with state regulations, and support both caregivers and clients.



Position Summary


The Client Care Supervisor is responsible for supervising client care activities, managing staff schedules, ensuring quality service delivery, and maintaining compliance with all state and agency policies. This position requires strong leadership, organizational skills, and the ability to build positive relationships with clients, families, and caregivers. The Supervisor will also participate in the agency’s on-call rotation to provide support outside of regular business hours.



Key Responsibilities



  • Supervise, support, and provide guidance to the case management team to ensure high-quality service delivery.

  • Track and maintain accurate records of new clients, discharged/lost clients, clients on hold, and pending.

  • Oversee the intake and onboarding process for new clients, ensuring smooth transitions and care plan implementation.

  • Collaborate with the scheduling department to ensure all authorized hours are staffed appropriately and in a timely manner.

  • Monitor client satisfaction, respond to concerns, and coordinate follow-up actions as needed.

  • Conduct regular audits of case documentation to ensure compliance with agency standards and regulatory guidelines.

  • Act as a liaison between the agency, caregivers, families, and clients to ensure seamless communication.

  • Coordinate interdisciplinary meetings or case conferences when needed to address complex client care needs.

  • Prepare weekly and monthly reports on client activity and trends for management review.

  • Support retention efforts by identifying risk factors for client drop-off and working with relevant departments to address them.

  • Conduct in-home visits, assessments, and supervisory visits as required by CT regulations.

  • Create, update, and manage caregiver schedules in Sandata, ensuring coverage for all shifts.

  • Ensure timely completion of prior authorizations and documentation.

  • Maintain compliance with CT DSS and state home care regulations.


  • Participate in the on-call rotation, responding to after-hours calls from clients and caregivers to resolve scheduling and client care issues.

  • Collaborate with agency leadership to improve operational efficiency and client satisfaction.

  • Hire, train, and manage personnel within your department.

  • Visit office locations where your team is location 1 or twice a week



Qualifications



  • Associate’s or Bachelor’s degree in Human Services, Healthcare Management, Nursing, or related field (preferred).

  • Minimum 2 years of experience in home care, healthcare, or case management required.

  • Previous supervisory or leadership experience strongly preferred.

  • Strong knowledge of CT state regulations governing non-medical home care.

  • Proficiency in scheduling systems (Sandata preferred) and Microsoft Office Suite.

  • Excellent communication, problem-solving, and organizational skills.

  • Ability to manage multiple priorities and meet deadlines in a fast-paced environment.

  • Compassionate and client-focused with a commitment to quality care.

  • Valid driver’s license and reliable transportation for home visits.
