Phoenix Home Care & Hospice is seeking Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide compassionate, skilled 1:1 care in the homes of our clients in Centralia, MO. Job Responsibilities:

• Follow the individualized care plan developed with the physician
• Monitor, document, and report patient conditions using electronic charting
• Educate and support the client and their family
• Participate in team meetings and required training

Qualifications:

• Active Missouri or Compact State Nursing License (LPN or RN)
• Compassionate and professional approach to care
• Strong communication and critical thinking skills
• Ability to work independently and manage time effectively
