Phoenix Home Care & Hospice is looking for compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) in Mexico, MO to provide 1:1 skilled care to clients in their homes. Your Role:

• Deliver personalized, skilled nursing care in a home setting
• Monitor and document patient conditions using electronic charting
• Communicate with families and educate them as needed
• Attend training and collaborate with the care team

Qualifications:

• Active LPN or RN license in Missouri or Compact State
• Strong communication and problem-solving skills
• Reliable, compassionate, and able to work independently
• Evening and/or weekend availability
