Client Service Representative - Patient Scheduler

The primary responsibility of a Client Service Representative (CSR) is to be a resource to the clients of Oak Medical (Skilled Nursing, Assisted Living, and Independent Living facilities) and the team of Physicians, Nurse Practitioners, Physician Assistants, and Registered Nurses. The team of CSRs creates the daily patient schedules for the team of Health Care Providers, ensures patient visits remain in compliance for Medicare/Medicaid, and acts a liaison between clients and Oak Medical.

Responsibilities

• Maintain the master log of patient appointments


• Answers calls from assigned facilities, sends messages to appropriate providers, follows up outstanding calls


• Utilize Microsoft Outlook to receive emails regarding facility censuses


• Manage patient charts within company Electronic Health Record


• Fax completed documentation of patient notes to various facilities


• Manage Physician, Nurse Practitioner, or Physician Assistant’s daily patient schedule


• Monitor medical record requests to ensure their timely arrival for new patient appointments


• Follow up on pending/incomplete notes to ensure timely completion of patient documentation


• Provide coverage for other teammates during vacation or days off

Desired Experience/Qualifications

• 2 years Unit Clerk or Health Unit Coordinator Experience


• Experience working within Skilled Nursing Facilities preferred
