Pharmacy from an accredited college of pharmacy (required) Area of study or major: Pharmacy Preferred educational qualifications: PharmD from an accredited college of pharmacy Position specific testing requirement: N/A Minimum Level of Experience Required: 1-3 years of job-related experience Prior job title or occupational experience: N/A Prior specific functional responsibilities: N/A Preferred experience: Meets the preceptor criteria as outlined by ASHP Other experience requirements: Completion of a post-graduate year 2 residency (PGY2) or fellowship -or- Completion of a post-graduate year 1 residency (PGY1) plus minimum 2 years of experience as a clinical specialist or equivalent role, including pharmaceutical care and pharmacotherapeutic monitoring -or- Minimum of 3 years of experience as a clinical specialist or equivalent role, including pharmaceutical care and pharmacotherapeutic monitoring Fluent in the use of drug information resources Ability to evaluate primary medical literature and develop evidence-based recommendations Ability to perform Medication Use Evaluations (MUE) and submit recommendations for system-wide changes Ability to provide pharmacokinetic and weight-based dosing Strong interpersonal, verbal, written communication, and problem-solving skills Ability to read and write printed material in English Ability to perform patient interviews and medication counseling effectively Ability to work well within a healthcare team Proficient with bedside aseptic technique Customer-service oriented Proficiency with computer keyboard operations Proficiency with Microsoft Office applications (preferred) Ability to troubleshoot and perform minor maintenance on pharmacy equipment, printers, computers, and telecommunication equipment, with adequate training Familiarity with pharmacy software Flexible scheduling Current licensure as a registered pharmacist by the State of Ohio Board of Pharmacy, or eligibility to become licensed within 60 days of employment start date Board Certified Oncology Pharmacist (BCOP) or completion within BPS-defined timelines to sit for examination Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications obtained within 90 days of employment start date
