The LPN is proficient in all tasks performed by clinical coworker staff. Additionally, the LPN acts as a role model, trainer, and mentor for clinic staff. Works cooperatively with Clinic Team Supervisors to coordinate and facilitate completion of work required to provide high levels of patient care and service. Performs duties and responsibilities in a manner consistent with our mission, values, and standards.
Essential Duties:

Coordinates with the Clinic Team Supervisor and Clinic Manager to identify and address opportunities to provide better patient care and service.
Assists with gathering and delivering information to team members in accordance with the patient centered medical home huddles.
Reviews patient charts and identifies gaps in patient care in accordance with the patient centered medical home model.
Determines work priorities and assist, as needed, to ensure timely work flow for patient care.
Assists providers with special tests and procedures within the standard scope of practice.
Assist in clinic with patient care as needed. Including, but not limited to, rooming patients, medication refills, referrals, and phone calls.
Completes medical prior authorization, as needed.
Triage patients in person and via telephone.
As needed to ensure good patient flow, the LPN will support patient care delivery by helping health care providers during examinations; prepare laboratory specimens, perform basic laboratory tests on the premises, dispose of contaminated supplies appropriately, sterilize medical instruments, administer medications on the premises, draw blood, prepare patients for x-rays, take electrocardiograms, remove sutures, change dressings, schedule referrals/outside diagnostics/testing appointments, administer immunizations/vaccines, maintain vaccine records, and schedule follow-up appointments.
Assumes self responsibility for all direct patient care activities, as indicated by the Registered Nurse (RN), within the scope of the LPN license.
Makes operational decisions as appropriate in the absence of the Clinic Team Supervisor and/or Clinic Manager.
Other duties as assigned.
Requirements
· Licensed Practical Nurse (LPN) in the State of Nebraska
· Certified – Basic Life Support
· 3 years LPN experience preferred
· 1 year clinic experience preferred
· Excellent problem solving skills
· Computer skills required
· Electronic Medical Record experience preferred
· Highly organized and ability to meet deadlines
· Attentive to detail
· Dedicated to quality and accuracy
· Self-starter and able to work with minimal supervision
· Ability to interrelate with wide variety of people
· Ability to maintain high level of confidentiality
