Job Description - Clinic Nurse, Advanced Lung Disease

Overview: The Advanced Lung Disease (ALD) Clinical Nurse Specialist serves as the clinical liaison for our ALD Program in the ambulatory setting. Under the direction of the ALD providers, the specialty nurse will coordinate clinical activities in accordance with orders, policies, and protocols.
Roles and responsibilities:
Management of incoming triage calls which include clinical assessments and management of symptoms and side effects of therapy. Perform nursing assessment to collect data and gather clinical information to assist patients. Schedule sick visits with providers as needed. Document appropriately in the medical record.

Provide compassionate and focused nursing support to patients, caregivers, and family members. Educate and provide educational materials to patients and caregivers.

Use clinical reasoning based on established evidence-based policies, procedures, and guidelines for decision-making.
Assist with new patient appointments by gathering documentation and diagnostic testing results prior to scheduled appointments. Coordinate outgoing referrals to outside services such as hospice, palliative care, and lung transplant.
Participate in planning activities and coordination for various pulmonary support groups, including for pulmonary hypertension and pulmonary fibrosis groups. Attend meetings with group members.
Coordinate scheduling of lab testing and medication titration for patients on high risk medications according to established protocols. Report any abnormal findings to ALD providers and provide patient follow-up and chart documentation.
Assist with new specialty pharmacy-provided medication enrollments, prior authorizations, insurance appeals, and financial assistance. Provide prescription refills as needed.
Represent Piedmont Healthcare to national nonprofit organizations and accrediting bodies, such as the Pulmonary Fibrosis Foundation and Pulmonary Hypertension Association. Participate on relevant committees and attend national meetings and conferences.

Qualifications:

MINIMUM EDUCATION REQUIRED:
Successful completion of an accredited registered nurse program OR bachelors degree in nursing.
MINIMUM EXPERIENCE REQUIRED:
None
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
State of Georgia Licensure as a registered nurse required.
