Job Description - Clinic Site Nurse (RN)

We are seeking a Clinic Site Nurse (RN) who is passionate about making a difference in our community to join our OB team. 


 


At Lone Star Circle of Care (LSCC) we strive for exceptional, equitable patient care that leads to healthier communities. The Clinic Site Nurse (RN) is responsible for providing professional nursing care and support to patients and families through direct and indirect patient care. This position provides clinical guidance to Licensed Vocational Nurses (LVNs) and Medical Assistants (MAs) as well as other non-licensed nursing personnel in the provision of patient care. This position participates as part of the integrated health care delivery team and will be required to float to LSCC facilities as needed. The Clinic Site Nurse (RN) collaborates with employees at all levels within the organization, must be able to maintain professional working relationships, and provide excellent customer service.


 


Monday - Friday, 8 am - 5 pm Work Schedule

A Day in the Life of a Clinic Site Nurse (RN) may look like this:



  • Perform comprehensive patient assessments, including triage and reassessments

  • Provide direct and indirect patient care based on clinical standards and best practices

  • Implement interventions safely, appropriately, and thoroughly document care provided

  • Performs care to meet the physical, psychological, emotional, and social needs of the patients/families based on the patient’s age, developmental stage, educational needs, and family/guardian expectations

  • Communicate patient and family concerns to the appropriate healthcare provider

  • Educate patients, families, and caregivers on health conditions and care plans in compliance with HIPAA standards

  • Manage referrals to specialty services and report barriers to the assigned Provider

  • Analyze clinical reports, data, and medical information to assess patient need

  • Collaborate with the interdisciplinary team to develop and adjust care plans

  • Interprets an extensive variety of technical instructions and manages several abstract and concrete variables

  • Participate in data collection and quality improvement initiatives under Clinic Leadership guidance

  • Perform clinical and non-clinical skills proficiently and in accordance with policies and procedures

  • Manage vaccine storage, inventory, and documentation in accordance with regulatory requirements

  • Coordinate with Pharmacy staff to maintain compliance and documentation for formulary management, sample inventory, and prescription assistance programs

  • Verify accuracy of medication administration by following all safety measures

  • Support monthly clinical program reporting as directed by Clinic Leadership

  • Mentor, supervise, and delegate tasks based on staff competency, documenting training and proficiency appropriately


 


We ask our Clinic Site Nurse (RN) to possess a minimum of:



  • Graduate of accredited nursing school

  • Current valid State of Texas Registered Nurse (RN) license

  • Basic Life Support (BLS) certification from American Heart Association or American Red Cross


 


The following experience/skills are preferred:



  • Previous work experience in outpatient/clinic nursing

  • Previous work experience using Electronic Medical Records (EMR)

  • Bilingual English/Spanish language skills
