Job Description - CLINICAL CARE COORDINATOR (Fetal Care Coordinator)
Summary
The Fetal Diagnosis and Treatment Center (FDTC) is a multi-disciplinary program that cares for mothers and their babies with congenital abnormalities. The FDTC is currently seeking an experienced nurse as a Clinical Care Coordinator, who will be responsible for coordinating all aspects of the patient?s care from the initial referral through the patient care experience at Michigan Medicine. The Clinical Care Coordinator will promote open communication amongst the multidisciplinary team and enhance collaborative relationships with referring physicians. They will assess and address both medical and psychosocial needs in a patient-centered manner. They will participate in the tripartite mission of the FDTC which includes clinical care, education, and research. In their cover letter, qualified RN candidates will provide a summary to describe their general knowledge of neonatal and/or obstetric conditions, as well as their potential for learning the fetal and neonatal surgical interventions and treatment offered at Michigan Medicine.

Responsibilities*
* Assesses health status and identifies needs of FDTC patients with specific focus on those requiring specialized antenatal fetal care, and neonatal surgical or intensive care.
* Reviews patient records and performs patient intake/follow-up in advance of patient clinical appointments in FDTC. Intakes may be via phone, virtual platform, or in-person
* Maintains complete and accurate patient records
* Facilitates the development and execution of comprehensive and individualized management plan
* Oversees care coordination for FDTC complex fetal patients, maintaining a comprehensive patient list
* Advocates for individual patient needs by incorporating patient values in care planning
* Utilizes resources across the health system to coordinate clinical care
* Respond to patient treatment-related phone calls and inbasket messages
* Assists physicians with clinical activities and procedures, as needed.
* Assists with the development of educational materials for specific disease diagnoses and program descriptions for patients and providers
* Provides education for parents/families and referring providers
* Provides phone and written follow-up for patients returning to their referring physician for care
* Provides professional support/education to staff involved with FDTC (such as physicians, sonographers, Pediatric surgery clinic staff, VVWH OB clinic, VVWH OR)
* Collaborates effectively with a multi-disciplinary team
* Utilizes strong critical thinking and clinical judgment to troubleshoot and problem-solve system barriers to clinical care
* Participates in collaborative research projects with multi-departmental staff
* Participates in data collection including FDTC registry and other QA/QI endeavors for the FDTC
* Maintains records and assists with subjects enrolled in clinical trials
* Supports the advancement of the FDTC enterprise through participation in education, research, outreach, and marketing
* Other nursing duties as assigned. This may include supporting the antenatal testing or fetal therapy CCC teams for unanticipated absences.

Hourly range for RSAM Registered Nurses $39.47 - $61.39 / hour
Evening Shift Differential-$3.00 / hour
Night Shift Differential- $4.00 / hour
Day Shift Weekend Differential- $2.90 / hour
Evening Shift Weekend Differential- $5.90 / hour
Night Shift Weekend Differential- $6.90 / hour
Charge Nurse Differential- $1.00 / hour

Required Qualifications*
Required Educational Requirement; applicant must meet one of the following:
Bachelors degree in Nursing OR
Associates degree or diploma in Nursing and a Masters degree or higher in Nursing
Current license to practice as a Registered Nurse in the State of Michigan.
Minimum of 5 (five) years within the last 7 (seven) years practice as an RN in an inpatient or outpatient perinatal setting, including complex high-risk antenatal care.
Participation in unit activities that demonstrate an ability to work collaboratively and positively contribute within a multidisciplinary team
Experience with patient and staff education
Highly motivated with demonstrated ability to function independently and as a team member
Must demonstrate expert communication, organization, and prioritization skills
Demonstrated accountability/dependability as evidenced, for example, by a stellar attendance record; by completion of mandatories and unit-based competencies within an established time frame; by meeting deadlines and completing required work
Demonstrated agility in managing rapidly changing situations effectively
Must possess excellent customer service skills
Competent computer skills
Demonstrated competency in fetal monitoring interpretation; successful completion of the AWHONN Intermediate Fetal Monitoring Course. Successful completion within 6 months.
Certification in inpatient obstetric nursing or inpatient antepartum nursing. Successful completion within 1 year of hire date.

Hours: Generally 7:30am- 4pm M-F 40 hours/week option with variable/flexible scheduling to accommodate unit needs
