Clinical Community Health Worker - Certified Medical Assistant

JOB SUMMARY

The Clinical Community Health Worker/Certified Medical Assistant supports the Lightpath Health program by providing proactive individualized coaching and resources linking to a group of patients through in-person and virtual methods with an end goal of affecting patient healthy living and behavioral change.

They will assemble and be content experts on a list of area community resources to improve the health of the managed population. The Clinical Community Health Worker/Medical Assistant will foster an environment of individual responsibility through patient education and support. Working closely with the Manager, the Clinical Community Health Worker is tasked with achieving system goals of improving health and clinical outcomes for patients by using timely and appropriate coordination of quality healthcare services to meet an individual's specific health needs to promote positive outcomes.

ROLE AND RESPONSIBILITIES

• Research, identify, and document list of area community resources and monitor up-to-date information.


• Support outpatient community health clinic and the Provider's needs by performing medical assisting duties, including but not limited to greeting and rooming patients, assisting patients with virtual connections, obtaining patient vitals, ROS, and history, reviewing plans of care or instructions with patient, conducting minor tests, and/or administering vaccinations.


• Administer health assessments to patients through in person or virtual methods.


• Provide needed follow up with the managed population related to chronic condition management, educational health information, appointment requests, and monitor patient communication pathways as needed.


• Work closely with Care Managers, Pharmacists, Social Workers, Dieticians, and other clinical Care Team members to support patient needs and the Provider's plan of care.


• Work closely with peers, management, other internal departments, leadership, physicians, volunteers, and community partners to support the Community Foundation program goals and initiatives.


• Complete data entry, patient documentation/record keeping, and reporting accurately and within required time frames according to program guidelines.


• Support off site community health and educational events through planning, set up, tear down, and staffing of events as needed.


• Support in creation and documentation of policies and procedures when needed.


• Ability to work with a diverse patient/family population, including both adult and pediatric populations.


• Provide superior service and always foster strong relationships with patients and their families/caregivers.


• Ability to establish and maintain effective and collaborative working relationships with clinical staff, coworkers, volunteers, vendors, and community partners.


• Ability to establish priorities and proceed with objectives without constant supervision.


• Ability to work independently, while collaborating with other team members both internally and externally at varying levels.


• Ability to interpret, adapt, and apply guidelines and protocols.


• Ability to self-motivate, prioritize, and be willing to invest in a change process to improve efficiency.


• Demonstrated knowledge and experience with behavior change, as well as self-management and motivational interviewing techniques.


• Excellent communication and interpersonal skills. Good oral, written, and presentation skills. Bilingual and Fluent (English/Spanish) Required.


• Knowledge of local community resources desired.


• Proficient computer skills including Microsoft Office Suite, data entry, report retrieval and generation; experience documenting in Electronic Health Records and utilizing telehealth applications preferred.


• Recognized and adhered to the scope/limitation of job requirements.


• Performs other related duties as assigned.

QUALIFICATIONS

• High School Diploma or equivalent required.


• Community Health Worker Certificate required or to be obtained within 90 days of employment.


• Medical Assistant Certificate highly preferred and may be substituted for similar clinical certifications, acumen or demonstrated experience.


• 1-3 years' experience in healthcare required.


• 2+ years health coaching, wellness, health promotion, health outreach experience preferred. Health education or weight management background a plus.


• CPR or BLS certification for Health Care Professionals required.


• Must maintain a valid driver's license and vehicle.

PHYSICAL REQUIREMENTS

• Must have reliable transportation and willing to travel within DFW area.


• Must have flexible working hours to accommodate weekend or evening scheduled events as needed


• Must be able to stand for various lengths of time.


• Must have stamina to perform manual duties that require strength, such as bending, standing, reaching, and lifting up to 30 lbs.


• Must be able to tolerate heat/outdoor working atmosphere
