Job Description - Clinical Coordinator Behavioral Health Liaison
Established in 1980, the Greater Lawrence Family Health Center, Inc. (GLFHC) is a multi-site, mission-driven, non-profit organization employing over 700 staff whose primary focus is providing the highest quality patient care to a culturally diverse population throughout the Merrimack Valley. Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites in Lawrence, Methuen, and Haverhill and is the sponsoring organization for the Lawrence Family Medicine Residency program.
The Clinical Coordinator / Behavioral Health Liaison (BHL) provides administrative, operational, and patient support to ensure efficient clinic workflows and timely access to behavioral health services. This role serves as a liaison between patients, providers and care teams, supporting scheduling, referral management and coordination of care.
Job Responsibilities and Performance Standards:
Operations & Administration
Provides administrative support to Behavioral Health Department leadership; manages calendars, meetings, and correspondence.
Maintains clinician schedules/templates to optimize access and workflow.
Prepares reports, meeting minutes, and maintains organized records and databases.
Coordinates supplies, invoices, and basic purchasing processes.
Supports recruitment activities, including interview scheduling and onboarding.
Behavioral Health Coordination
Serves as primary contact for behavioral health services at assigned site(s).
Manages BH referrals, including EMR work tasks, scheduling, tracking, and authorizations.
Facilitates warm handoffs and coordinates internal/external referrals and transitions of care.
Conducts appointment reminders, manages no-shows, and backfills schedules.
Identifies barriers to care and connects patients to appropriate resources.
Patient & Team Support
Communicates with patients, providers, and external partners to ensure continuity of care.
Participates in team meetings and quality improvement initiatives.
Provides information on behavioral health and community resources.
Maintains confidentiality and compliance with HIPAA; demonstrates cultural competence.
Clinical Support
Assists with basic clinical workflows (vitals, screenings, documentation, care gap measures) per protocol.
Additional Duties
Attends required trainings and meetings; performs other duties as assigned.
Qualifications:
Experience
Minimum 3 years of administrative and/or patient care experience, preferably in healthcare.
Strong organizational, communication, and problem-solving skills.
Proficient in Microsoft Office and EMR systems.
Ability to multitask in a fast-paced environment.
Bilingual (English/Spanish) required.
Education
High School Diploma or GED required; Human Services or healthcare training preferred.
Medical Assistant and CPR certification preferred (if performing clinical duties).
