Position Summary: This dynamic office-based role involves coordinating and overseeing all clinical aspects of patient care for the Rehabilitation team, ensuring adherence to established standards and protocols under the direction of a Director of Rehabilitation and/or Clinical Supervisor. This position is the heart of daily operations, interacting regularly with front-line clinicians and collaborating with various stakeholders—including clinical leadership, operations, interdisciplinary teams, patients, families, and physicians—to ensure seamless delivery of care. As a Clinical Coordinator, you play a crucial role in delivering high-quality, compassionate care to patients in their homes, making a meaningful impact on their health and well-being every day.

Essential Duties and Responsibilities:

  • Monitor communications in the clinical team communication platforms and provide answers or resolve issues. Escalate to leadership as appropriate.
  • Address patient complaints as an initial point of contact when receiving calls from Patient Service Representatives. If escalation is necessary, escalate to leadership.
  • Keeps track of recurring adverse situations and makes recommendations to leadership for additional educational opportunities for rehab clinical team members
  • Assist rehab clinical team by contacting and communicating with patients, referral sources and/or primary care physicians on the behalf of therapy clinicians as appropriate
  • Supports the leadership with administrative tasks such as adding approved PTO to calendar platforms and attendance tracking for quarterly all therapy meetings
  • Assists leadership with managing clinician productivity, availability and schedule related issues. Escalation of issues to leadership if necessary
  • Reviews utilization for therapy services (under or over utilization) and escalates those not inline to the leadership to address
  • Assists leadership with weekly report of delayed charting and remind clinicians to complete past due tasks
  • Attends daily huddles to be apprised of priorities and day to day operations

Requirements

  • CA Licensed Physical Therapy Assistant (PTA), Licensed Vocational Nurse (LVN) or higher discipline
  • Preceptor and/or leadership experience highly desired
  • Minimum of 2 years of clinical experience in home health or a related setting required
  • Strong clinical assessment and care planning skills.
  • Excellent communication and interpersonal abilities.
  • Demonstrate leadership capabilities and the ability to work effectively in a team environment.
  • Knowledge of Medicare regulations and OASIS documentation requirements.
  • Proficiency in electronic health record (EHR) systems and other applications.
