Clinical Director – Home Care Services
Syracuse, NY

Salary Range: $85,000–$100,000, based on experience

NurseCore is seeking a Clinical Director to lead home care services in Syracuse, NY. This full-time role combines clinical expertise and leadership to ensure safe, high-quality patient care in compliance with New York state and federal regulations.

Responsibilities:

Supervise nursing staff and Home Health Aides, ensuring training and competency
Manage client care: assessments, care plans, recertifications, coordination, and discharge planning
Oversee referrals, staffing, recruitment, onboarding, and scheduling
Conduct home visits, clinical reviews, and PAG meetings
Address client concerns and maintain accurate documentation

Qualifications:

Active RN license in New York
2+ years RN experience, including 1+ year in home health
Leadership experience supervising staff, including hiring/firing
BSN or 4-year degree preferred
Strong problem-solving and communication skills

Schedule & Travel:

Full-time, Monday–Friday, 8 a.m.–5 p.m.; occasional evenings/weekends
Local travel; occasional out-of-area/overnight travel
