💰 Salary: $90,000 – $115,000 + bonus potential

Role Overview

We are hiring a Clinical Liaison (RN) to support the launch and growth of a new hospital-partnered service line. You will act as the face of myLaurel inside the hospital, building trust, identifying eligible patients, and ensuring smooth transitions into our home-based acute care programs. This hybrid clinical–strategic role is ideal for a nurse experienced in inpatient hospital care who excels in relationship-building and care coordination.

Key Responsibilities

• Build strong relationships with hospital staff, physicians, and leadership

• Identify eligible patients and manage referrals into myLaurel programs

• Educate patients and caregivers on services and care expectations

• Collaborate with hospital staff to integrate workflows

• Track referral activity, patient outcomes, and satisfaction

• Support strategic growth of the new service line

Requirements

• Current, unrestricted RN license

• Floor Registered Nurses Only with hospital experience (ICU/ER preferred)

• 5+ years of inpatient clinical experience

• Strong care coordination, discharge planning, and communication skills

• EHR proficiency; BSN or related degree preferred
