Clinical Manager

Our client is looking for a Clinical Manager to lead how patients are assessed, admitted and cared for in a home health setting. You will decide what care each referred patient needs, set up their plan of treatment, and guide the field clinicians who deliver that care. Your work helps patients get the right level of care at the right time, and keeps care safe, compliant and well documented.

About the role

You will report to the Director of Patient Services. You will assess and plan care for patients referred to the agency, and oversee clinical field staff to support quality care, case management and timely, accurate documentation. The role involves a moderate amount of travel to assess patients in institutional settings and in their homes. Your routine will often change, and you will take part in the on-call supervisor rotation.

What you will do

• Screen and assess the home health needs of referred patients, and complete intake within set timeframes


• Tell the referring party early if a patient will need an alternative discharge plan


• Work with physicians, nurses and discharge planners to decide the right level of care


• Make sure each patient has an individualized plan of treatment that follows physician orders, patient needs, insurance guidelines and agency protocol, including formal and informal supports


• Arrange equipment, medical supplies and ancillary services, and make sure they are documented


• Schedule and assign visits for field staff, and delegate tasks to support staff with clear, timely direction


• Contact managed care personnel to obtain authorizations when needed


• Lead case conferences to coordinate care and plan discharges on time


• Review admission and recertification documentation and physician orders to confirm each care plan fits the patient


• Act as a point of contact for anyone seeking information from the agency, and draw on community resources to meet patient needs


• Apply your knowledge of State and Federal regulations and of Medicare, Medicaid, managed care, charity care and other payor requirements


• Take part in quality and performance improvement activities and in orientation for new staff

What you will bring

• A current registered nurse license in the state


• At least three years of nursing experience


• Knowledge of OASIS and clinical documentation standards


• Clear verbal and written communication skills


• Strong problem-solving skills and the ability to juggle multiple responsibilities and set priorities


• A strong commitment to customer relations, with respect for patients, families, referral partners and colleagues from all backgrounds


• The ability to work independently, make sound decisions and keep patient information confidential

Preferred

• A BSN


• Two years of home health nursing experience

Working conditions

• Moderate travel to patients' homes and institutional settings


• Frequent changes to your routine


• Mostly seated work, with occasional standing, walking and bending


• Regular computer use, which can cause eye strain
