About the Role As our Home Health Clinical Manager, you’ll lead a team of skilled clinicians and support staff, ensuring care is delivered with excellence, efficiency, and heart. You’ll oversee day-to-day clinical operations, mentor staff, and serve as the key connection between field teams, support departments, referral sources, and leadership. You’ll split your time between direct field supervision and office-based oversight, ensuring our Key Performance Behaviors (KPBs) drive both outstanding patient outcomes and operational success.
Key Responsibilities
Apply evidence-based standards of care and lead practice improvement initiatives
Set clear performance expectations, provide tools and training, and support staff growth while ensuring accountability
Oversee hiring, onboarding, and development of Home Health staff; manage performance and address issues as needed
Monitor staffing ratios, schedule appropriately, and allocate resources efficiently
Partner with Team Assistant(s) to coordinate patient visits and staff assignments for timely care
Provide patient visits when needed to ensure coverage and continuity
Serve on the QAPI Committee; implement quality initiatives in the field and monitor compliance
Promote safe work practices for both staff and patients
Represent the Home Health program in the community and within the industry
Act as clinical leader in the Director of Clinical Services’ absence
Qualifications
Current Montana RN, PT, or OT license
Bachelor’s degree related to licensure
2+ years of home care experience and at least 1 year in a supervisory role
Strong leadership, communication, and problem-solving skills
Preferred: Professional certification
