What You’ll Do

• 
  

  Oversee all patient care operations and clinical staff

  
  


• 
  

  Coordinate assignments, referrals, and care plans

  
  


• 
  

  Ensure patients’ needs are continually assessed and documented

  
  


• 
  

  Support and mentor nurses, therapists, and aides

  
  


• 
  

  Partner with physicians and families to deliver the best possible outcomes

  
  


• 
  

  Champion QAPI initiatives and compliance standards

  
  


• 
  

  Lead with compassion, accountability, and vision

  
  

What It Takes

• 
  

  Active Registered Nurse (RN) license in Pennsylvania

  
  


• 
  

  Graduate of an accredited nursing program

  
  


• 
  

  3+ years of home health experience (preferred)

  
  


• 
  

  2+ years in leadership or management (preferred)

  
  


• 
  

  Current CPR certification

  
  


• 
  

  Strong organizational and communication skills

  
  


• 
  

  A reliable vehicle and valid driver’s license
