Job Description - Clinical Pharmacist

Description

 

Summary: The Clinical Pharmacist is responsible for providing comprehensive medication

management services for Southwest Community Health Center. This role integrates direct patient

care, provider collaboration, and population health initiatives to optimize medication therapy

outcomes. The Clinical Pharmacist serves as the program lead for the 340B Drug Pricing Program

and oversees medication-related inquiries and prior authorization processes to ensure safe,

effective, and cost-efficient patient care.

SPECIFIC DUTIES AND RESPONSIBILITIES:   All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA). 

Clinical Care & Medication Management

· Provides comprehensive medication management (CMM) under collaborative practice

agreements (as applicable).

· Conducts medication reconciliation and medication therapy reviews.

· Consults on adjusting medication regimens per protocol and evidence-based guidelines.

· Provide patient education on medication use, adherence, side effects, and cost-saving

options.

· Supports transitions of care and reduce medication-related hospital readmissions.

340B Program Oversight

· Serves as the designated lead for the 340B Drug Pricing Program.

· Educates providers and staff on the 340 B program.

· Ensures full compliance with Health Resources & Services Administration (HRSA)

regulations.

· Maintains policies and procedures related to 340B operations.

· Conducts internal audits and oversee contract pharmacy relationships (if applicable).

· Monitors inventory management and prevent diversion or duplicate discounts.

· Collaborates with finance and administration to optimize cost savings and reinvestment

strategies.

· Serves as primary liaison during HRSA audits.

Prior Authorizations

· Oversees and streamlines the prior authorization process.

· Assists providers and staff with clinical documentation to support insurance approvals.

· Conducts peer-to-peer reviews when appropriate.

· Identifies trends and implements workflow improvements to reduce delays in care.

· Collaborates with payers to optimize formulary access.


Requirements

 Required Education:

  •   Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy. 

Required Licensure: 

  • Active, unrestricted pharmacist license in state of Connecticut. 

Required Experience: 

  • 3 years of experience as a pharmacist in a clinical setting. 
  • Good written and verbal communication skills. 
  • In depth knowledge on drugs, medical procedures and therapies, diagnosis and disease states, pharmacodynamics, and pharmacokinetics. 
  • Skills in working with technical automated equipment and information systems 

Strongly Preferred:

  •  Completion of PGY-1 residency 
  •  Board certified or board eligible 
  •  Experience in a Federally Qualified Health Center 
  •  Experience managing or auditing a 340B program
