Job Description - Clinical Pharmacist I

Position: Clinical Pharmacist
Department: Inpatient Pharmacy
Schedule: Full Time, 40 hours (Day, Evening, Night)

POSITION SUMMARY:

Under the direction of the Central Team Leader and Clinical Manager, interprets physician medication orders. Compounds and dispenses medications and other pharmaceuticals. Acts as drug information resource to patients, medical staff, nursing staff and ancillary department personnel. Ensures optimum patient outcomes related to medication use by preventing or resolving drug-related problems and coordinating the acquisition, distribution and control of all drug therapy to patients under the care of the pharmacist.

ESSENTIAL RESPONSIBILITIES / DUTIES:

Ensures all drugs and medications are handled in the manner established by the Pharmacy and Therapeutics (P & T) Committee.

Monitors drug therapy by direct patient contact, chart review, reviews applicable laboratory data to identify drug-related problems or to prevent drug-related toxicity. Specific aspects of drug therapy evaluated include: clinical indication for drug prescribed, appropriate dose and route of administration for indication and organ function, absence of therapeutic duplication, screening allergy history and current medications, use of formulary drugs or less costly drugs when possible. Evaluates pharmacokinetics (drug effects) and assesses drug or nutrient interactions.

Collaborates with Clinical Specialists, Physicians and other medical staff on drug therapies and regimens. Reviews and approves medication orders, and participates in daily rounds with the medical team. Enters physicians' orders into the inpatient pharmacy system.

Provides timey and accurate drug information to patient, nursing and medical staff. Adjusts dosages for proper renal function, and accordingly to the age and weight of the patient, etc. Conducts interventions on IV to PO conversion, and therapeutic interchange, as needed.

Assures accurate and timely acquisition, distribution and control of all medications, including investigational drugs.

Maintains all protocols and policies related to restricted drugs and narcotics.

Ensures assessment and interventions of patients is systematic, complete, timely and documented according to standards (timely, legible)

Supervises the activities of supportive personnel including student interns and technicians. Responsible for checking all medication and sterile products prior to dispensing and Pyxis refill deliveries. Assists in the orientation and training of pharmacy residents, students and technicians, as well as newly hired pharmacists.

Conducts at least two in-service education programs to nurses or physicians at Boston medical Center.

Develops goals for each fiscal year in conjunction with the manager(s). Works on accomplishing goals throughout the year.

Participates in the Adverse Drug Reaction (ADR) Reporting Program, Drug Use Evaluation (DUE) Program, etc. Participates in departmental and hospital-wide quality assurance activities. Takes corrective action to improve performance on problem areas.

Ensures compliance with standards and regulations from federal, state and local governments, as well as other regulatory bodies such as The Joint Commission. Resolves patient complaints emanating from areas of responsibility. Ensures that all significant patient complaints are reported promptly to the Patient Advocate or Director of Quality Improvement.

Maintains own professional and specialized competencies, including the following:

Assumes responsibility for participating in experiences necessary to ensure appropriate skill development and training.

Maintains and displays appropriate knowledge of developments in the field through continuing education, reviewing pertinent literature and/or by attending professional meetings.

Attends and participates in hospital and other management training and development workshops.

Follows all department policy and procedure, to include:

Ensures that all staff attend mandatory BMC training sessions, receive annual TB testing, etc.

Maintains all designated hospital information confidential at all times.

Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided.

Utilizes hospital's Values as the basis for decision making and to facilitate the hospital mission.

Follows established hospital infection control and safety procedures.

Performs other related tasks as needed.

JOB REQUIREMENTS
EDUCATION:
PharmD degree ( or Bachelor's plus 2 years hospital pharmacy experience).

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
Work requires licensure to practice pharmacy in the state of Massachusetts, or eligibility to become fully licensed within 45 days from date of hire.

EXPERIENCE:
Prior hospital pharmacy experience preferred but not required.

KNOWLEDGE AND SKILLS:

Work requires a comprehensive understanding of the procedures, systems, and equipment used by pharmacists and technicians in the pharmacy as well as a high level of proficiency in performing pharmacist's duties and responsibilities.

Work requires a high level of analytical ability, organizational and personal management skill and focus to ensure the concerted approach to operations.

Practice experience working with as well as revising and/or implementing information systems.

Excellent interpersonal and telephone communication skills. Professional in conduct and demeanor.

Proficient with standard software used in a pharmacy office setting (i.e. MS Word, Excel, pharmacy systems, web browsers, etc.)
