What You'll Do:

• Serve as an LPN overseeing the Resident Assessment Instrument (RAI), care planning, and PDPM processes to ensure compliance and support optimal resident care (required)
• Conduct and review MDS assessments and manage OBRA and Medicare schedules
• Support interdisciplinary care planning
• Participate in utilization reviews and quality assurance efforts
• Work collaboratively with a highly successful and supportive team

What You'll Need:

• Licensed Practical Nurse (LPN) in good standing in the State of Florida (required)
• Three to five years of related experience and/or training, or equivalent combination of education and experience
• Minimum of three years of experience in SNF MDS management (required)
• Proficiency in Microsoft Office, including Word, Excel, and Outlook (required)
• Previous experience with RAI, care planning, and PDPM processes (required)
• Strong verbal, written, and electronic communication skills, along with excellent organizational and teamwork abilities
