Clinical Resource Liasion

SUMMARY:

The Clinical Resource Liaison (CRL) works under the guidance and supervision of the Registered Nurse Case Manager (RNCM) as well as other disciplines at Clinical Managers (CL-M) discretion.  The CRL supports the delivery and coordination of care to participants by assisting the clinical team.

SPECIFIC DUTIES AND FUNCTIONS:

• Facilitate communication through answering direct nursing phone calls to assist with nonclinical, non-urgent, direction of calls to appropriate team member.
• Collect and transport urine and stool specimens from community (when LPN not available)
• Daily rounding in the day health center
• Deliver urgent nonskilled supplies (diabetic, incontinence, etc)—no meds
• Other related duties as assigned
• Assist nurse with procedures (eg- dressings)
• Assist with facilitation of virtual visits for participants
• Wellness- social calls/appointment reminders
• Attend afternoon appointment review meeting; assist facilitation of rescheduling missed appointments; communicate with transportation team
• Assist with retrieval and sending records, faxes
• Collect/maintain information
• Attend home visits with clinical team for assistance as needed
• Update Grand Pad with appointment reminders
• Ordering supplies/DME for RNCM (PAP, Oxygen, incontinence, etc)
• Screening forms/pre procedure coordination
• Transportation coordination and requests

KNOWLEDGE, SKILLS AND ABILITIES:

• Excellent customer service and communication skills including face to face, telephone, virtual, and written abilities
• Familiar with medical terminology and recognize urgent medical issues
• Health care experience working with geriatric population (ie- PCA, CNA, nursing student, MA, or EMT, patient advocate, social work, long term care, hospital, or ambulatory care
• Skillful in Microsoft Office
• Able to navigate through electronic medical record and other participant- oriented software
• Document in the EMR as needed
• Ability to flex to the needs of the participant and or center/clinical teams needs
• Basic Life Support/CPR

• This position would be mostly center based, but may require some in home, clinic, day health center, office.
