The Clinic Service Clerk/ACA performs Service Scheduling of Clinic physicians, PACE Nursing and responsible for orders of clinic supplies, glucose monitoring, vital signs and Participant Care Activities and Personal Care and other tasks as assigned to assist the nursing staff.

1. Performs Phlebotomy as a Licensed Phlebotomist
   1. Performs Phlebotomy for all participants scheduled according to the scope of her license and training.
   2. Utilizes universal precautions at all times.
2. Performs Participant Scheduling in electronic health record and Schedules all participant's laboratory tests, assuring the lab work is scheduled prior to the medical care visits, in order for lab work to be available for the visit.
   1. Schedules all Participants with the Primary Care Physician and Nurse Practitioner for medical care visits; coordinating the visits with the 6 month and annual assessments of all participants.
   2. Performs Vital Signs and Glucose Monitoring for Participants under supervision of Clinic Staff
   3. Performs glucose monitoring and vital signs in an accurate manner.
   4. Reports abnormal findings to clinic staff.
   5. Utilizes universal precautions at all times.

3.  Assist Participants with Activities of Daily Living (ADL), to keep them at their highest functioning level and minimizing further deterioration.a. Performs Basic

a. ADL Care of designated Participants, including but not limited to; transferring, bathing, dressing, catheter care, basic skin care, incontinent care, and toileting as assigned.

b. Performs meal functions which can include the following:  Oral feeding of Participants.

c. Reports significant observations to supervisors and communicates Participant changes to team members.

4. Follow Participant Rights guidelines including confidentiality at all times to ensure that participants are being treated with dignity and respect as adults within the PACE environment.

a. Communicates effectively with participants, families, staff, other departments and agencies.

b. Reports significant observations to supervisors and communicates participant changes to team members.

c. Participate in the coordination of 24-hour care delivery and reports to a supervisor any inappropriate event or action to a Participant.

4. Assists in the Maintenance of the Unit to provide for a pleasant efficient and safe environment.

a. Orders supplies, tracks supplies and ensures adequate inventory.

b. Maintains laundry and disinfecting of units as scheduled on a routine basis.

c. Maintains clear pathways for Participants and assists with processes when Participants arrive and depart on the transportation vans.

d. Performs environmental rounds as scheduled.

Experience - 1-year exp with frail/elderly population
Education - HS Diploma or Equivalent
Licensure/Certification:  C.N.A, BLS Certification and Phlebotomy Certification.
