JOB SUMMARY

Under the supervision of the Clinical Supervisor of the Citywide Minna Team, the Bilingual Spanish Speaking Clinical Social Worker I/II (CSW I/II) works with formerly homeless, justice involved adults who have co-occurring mental health and substance use disorders. The CSW I/II will carry a caseload of residents from the supportive transitional housing site at 509 Minna St (Minna Project) and provide treatment including; individual and group psychotherapy, clinical case management, crisis intervention and 5150 assessments, substance abuse treatment, assertive outreach, intake, assessment, service planning and consultation.  The CSWI/II will coordinate closely with Westside Community Services (property management and forensic case management on site), support service partners, justice system partners (SF Superior Court, Adult Probation Department, Parole), and other Citywide Minna Team members to develop successful treatment strategies. Additional duties include advocacy and coordination with outside agencies, clinical documentation in the Epic Electronic Health Care Record, referrals and linkage, and other duties as assigned. The overall goal of the Minna Project is to help safely reduce the jail population, increase positive health outcomes, reduce homelessness, and foster the skills/supports necessary for clients to successfully exit the justice system. Service Provision

Provides ongoing mental health services, including psychotherapy (Individual, Group and Family), crisis intervention, and intensive outreach and engagement to a caseload of formerly homeless clients living at 509 Minna Street. Help client stabilize, maintain transitional housing and link to longer-term ongoing mental health, substance abuse, medical services and other programs. Make clinically appropriate discharge plans and terminations.

Intake, Assessment, Service Planning & Consultation

Coordinate services closely with Westside property management/forensic case management staff and the Citywide Minna Team to develop successful treatment strategies. Conducts intake of new clients, including assessment, community functioning, and service needs, when appropriate. This entails assessment of presenting problems, obtaining psychosocial history, conducting a mental status examination, and diagnosing (DSM V). Provides consultation to multidisciplinary team including partner agencies involved in client care.

Documentation and Administration

Track client contacts, assessments, and care plans. Utilize Medi-Cal billing for services. Maintains client records according to City and County and Medi-Cal requirements. Records data in client’s charts. Meet program productivity standards. Assists in processing quality improvement requests. Completes documentation of services in a timely fashion per Medi-Cal guidelines.  Attend staff and other assigned meetings, and participates in training sessions (e.g., In-services, grand rounds). Identifies program problems and makes recommendations to supervisory and administrative staff. Other duties as assigned.

Qualifications

Required: 

  • CSW I: Masters degree in social work, psychology or related field from an accredited university
  • CSWII: Masters degree in social work, psychology or related field from an accredited university and possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science and knowledge and abilities essential to the successful performance of the duties assigned to the position             
  • Must be registered with the California BBS as an ASW/AMFT/APCC or LCSW/LMFT/LPCC        
  • Knowledge of psychiatric service operations and ability to apply concepts in an acute care setting.        
  • Strong interpersonal skills and experience working with populations that is diverse in gender, sexual orientation, race, and ethnicity.            
  • Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines.  Ability to follow oral and written instructions accurately; detail oriented.  Able to efficiently prepare written summaries of activities, set priorities and complete reports within required time frames.  Knowledge of medical and/or psychiatric terminology.     
  • Ability to ensure patient care service standards are consistently met; assists staff with managing challenging patient interactions with sensitivity and skill.      
  • Spanish language fluency adequate to prove therapy to clients in this language           
  • The ability to do outreach to client units and in the community at inpatient hospitals and other community locations.                                 

 

Preferred: 

  • Two years working with severely mentally ill adults, formerly homeless, and dual diagnosed populations.           

 

Licenses required:        

  • Must be registered with the California BBS as an ASW/AMFT/APCC or LCSW/LMFT/LPCC
