Description

Under the general supervision of the Director of Clinical Services and as part of a multidisciplinary treatment team, the inpatient social worker’s duties include but are not limited to psychosocial assessment, case management, individual and family therapy and crisis intervention for patients hospitalized on an acute psychiatric unit. The social worker will provide timely and appropriate discharge planning taking into consideration the patient’s individual goals, preferences, needs and resources. 

DEPARTMENT DESCRIPTION:

Langley Porter Psychiatric Hospital

UCSF Langley Porter Psychiatric Hospital – Mount Zion serves as a location for our inpatient, partial hospitalization and intensive outpatient programs. The UCSF Inpatient Program is a 30-bed mental health service caring for adults with urgent behavioral and emotional concerns who are referred by a mental health care provider. The UCSF Partial Hospitalization Program and Intensive Outpatient Program are intensive, short-term treatment options for patients with moderate to serious behavioral and emotional conditions.

Direct patient care:  Maintain a caseload of patients hospitalized on the acute psychiatric unit for adults and provide psychosocial assessment, case management, individual patient/family meetings, treatment planning and discharge planning.   Provide psycho-education to patients and families. 

 

Communicate effectively and maintain positive relationships with community partners including SFDPH, Conservatorship Office etc. . 

 

Maintain up to date knowledge of the full the spectrum of community resources , including referral processes, for discharge planning needs including basic needs (food, shelter), long term locked psychiatric facilities, residential care facilities and outpatient programs, clinics and providers. 

 

May facilitate  psychoeducation groups 

 

 

 

Clinical consultation to a multi-disciplinary treatment team; facilitate referrals and services as needed; participation in ongoing assessment of treatment services including ongoing evaluation of the need for change and improvements; participate in the training/education of medical students, residents and/or other post-graduate trainees.  The incumbent may also be called upon to provide individual psychotherapy, outreach, and case management as indicated by varying programmatic and patient care needs.

Attend staff meetings, other duties as assigned. 

This includes maintaining knowledge of clinical service standards/regulations by LPPH, TJC, Medi-Cal and private funding.  Maintain current knowledge of infection control issues, national patient safety regulations, and implement practices including CDC hand washing guidelines.  Exercise sound judgment in taking sick leave to avoid spreading patients and co-workers to contagious illness.  Utilizes LEAN Management systems to include participation in huddles, True North Board 

leader rounding to address key performance indicators such as Patient Experience, Our People, Strategic Growth and Financial Strength.   

Participate in facilitation of Patient Advisory or Focus Groups on behalf of LPPH

Qualifications

MINIMUM REQUIREMENTS:

  • Master’s degree in social work, psychology, or related field from an accredited university 
  • For CSW2, eligible to register as an associate with the Board of Behavioral Sciences in the state of California 
  • For CSW3, Licensed with the Board of Behavioral Sciences in the state of California (LCSW, LMFT)
  • Minimum 3 years relevant experience
  • Recent direct patient care experience in a mental health setting, group and crisis management skills.
  • Demonstrated ability to provide transitional, comprehensive mental health services including psychotherapy, crisis intervention, outreach, and case management to a caseload of severely mentally ill adults who are high risk for psychiatric hospitalization. 
  • Demonstrated ability to complete psychosocial/functional assessments, mental status examinations, comprehensive treatment development, and discharge planning. 
  • Excellent documentation and clinical skills. 
  • Ability to prioritize; work independently, excellent follow-up, work well under pressure and deadlines. 
  • Demonstrated excellent attendance and reliability. 
  • Demonstrated ability to assess suicide and violence risk. 
  • Demonstrated ability to work harmoniously with colleagues as a member of the treatment team. 
  • Excellent communication and interpersonal skills. 
  • A preliminary job offer and assignment may be extended pending successful background clearance. 
  • This position requires flexibility to orient and work at all UCSF Medical Center locations.

 

PREFERRED:

  • Experience with populations diverse in ethnicity, gender, and sexual orientation. 
  • Experience with severely mentally ill.

 

LICENSES/CERTIFICATIONS:

  • For CSW2, eligible to register as an associate with the Board of Behavioral Sciences in the state of California 
  • For CSW3, Licensed with the Board of Behavioral Sciences in the state of California (LCSW, LMFT)
