Position: Social Worker (MSW)

Company: Home Health Care, Inc.

Compensation: $41.50 – $50.00 per visit + mileage reimbursement

Schedule: Full -Time Hours Monday - Friday

What You’ll Do:

• Provide in -home visits supporting patients and families.

• Deliver counseling, care planning, and crisis intervention.

• Connect patients to community resources and support systems.

• Collaborate with an interdisciplinary care team.

What You Bring:

• Master’s Degree in Social Work (MSW) required.

• Active LGSW or LISW license in Minnesota.

• At least 1 year of clinical Social Work experience.

• A passion for independent, relationship -based care.
