Job Description - Clinical Supervisor/Registered Nurse (RN) - Home Care

Responsibilities:

As a Clinical Supervisor Registered Nurse for Home Care you will plan and work with the Clinical Director and Branch Director to coordinate, provide and evaluate home care services focused on patients' individual needs. This is a critical management role and involves supervising the field staff for home care and private duty as well as supporting the business development plans of the agency.

Taking, reviewing and converting home care referrals
Assessing patients, developing plans of care, coordinating services and discharge planning
Completing client admission and pre-admission visits
Ensuring compliance with agency’s policies and local, state and federal laws and regulations
Directing and supervising all client care services
Completing required paperwork for insurance reimbursement and clinical record documentation
Serving in a rotating on-call schedule for clinical troubleshooting and support
Recruiting, interviewing, selecting, orienting, and supervising field staff
Evaluating and conferring with staff regarding performance
Representing the agency in the community
Other duties as assigned

Qualifications & Requirements:

Passion for delivering high-quality, patient-focused care
Strong leadership and communication skills
Understanding of home health regulations and insurance/reimbursement criteria
Current RN license in Nevada
2+ years of recent RN experience
1+ year of Gtube experience (plus traches and/or vents preferred)
1+ year in home health care
1+ year of supervisory experience
Bachelor’s degree from a National League of Nursing-accredited college preferred
