Overview
We’re seeking a dynamic Clinical Team Lead to provide mentorship, clinical support, and daily coordination for our home health care teams. This role is pivotal in maintaining quality care delivery while ensuring our clinicians feel supported and empowered in the field. The Clinical Team Lead acts as the go-to resource for staff nurses and helps bridge communication between clinical operations and leadership.

What You’ll Do

• Lead Field Support

  • Offer clinical guidance and real-time support to RNs and LPNs across patient caseloads.

  • Assist in assigning visits and balancing team workloads to maintain optimal patient care timelines.

  • Facilitate weekly case reviews, helping teams problem-solve and coordinate care.

• Support Team Performance

  • Be a point of contact for staff needing help with clinical decisions, patient situations, or care logistics.

  • Welcome and train new team members, providing ongoing mentorship and feedback.

  • Regularly check in with staff to encourage professional growth and address any concerns.

• Coordinate Across Departments

  • Serve as the link between clinical and administrative teams to improve workflow and communication.

  • Ensure timely documentation and updates to physicians, care partners, and internal systems.

• Monitor & Improve Quality

  • Track performance indicators like hospital readmissions, missed visits, and documentation timeliness.

  • Partner with leadership to address care gaps and participate in QAPI initiatives.

  • Support adherence to agency policies and home health compliance standards.

Qualifications

• RN license in the state of Missouri

• At least 1 year of recent home health experience

• Strong organizational and leadership abilities

• Working knowledge of EMRs and CMS home health regulations

• A collaborative, proactive approach to patient-centered care
