Job Summary

Perform a full range of professional nursing services, including assessing, planning, evaluating, and coordinating patient care in accordance with the team treatment plan. Provide supervision and support for Community Health Workers (CHW) during the provision of care coordination activities.

Requirements:

• Minimum two (2) years’ experience in clinical care setting
• Current New Mexico RN License
• Current CPR certification/training; ongoing maintenance of CEU's.
• Must pass a criminal background check
• Maintain a current driver's license, current automobile insurance, and maintain a clean driving record.
