OVERVIEW
Provides behavioral health assessment, therapy, crisis intervention, and care coordination services for patients experiencing mental health concerns. This role works collaboratively with patients, psychiatrists, nurse practitioners, and community resources to develop individualized treatment plans, support recovery goals, coordinate discharge planning, and deliver compassionate, patient-centered behavioral health care.
Schedule: 36 hours/week, Monday-Friday
Shift: Days
Location: Mercyhealth Henry Palmer Building, Janesville, WI
Pay Range: $30.59/hr - $53.41/hr; pay is based on licensure level
ESSENTIAL DUTIES AND RESPONSIBILITIES
Completes psychosocial assessments and treatment plans in cooperation with patient and multidisciplinary team.
Conducts a comprehensive assessment of the clinical needs of each patient prior to beginning individual, family or group therapy.
Works collaboratively with the patient to develop a treatment plan addressing patient’s identified treatment goals.
Delivers behavioral health care in accordance with patient evaluation, treatment plan and within the scope of their license.
Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist and or NP.
Provides crisis assessment and intervention.
Consults and coordinates care of patient with physician and or NP, therapists, and community agencies.
Provides appropriate discharge planning.
Refers patient or patient's family to other community resources.
Participates in clinical collaboration and or psychiatry supervision to discuss cases and determine appropriate course of treatment.
Maintains pertinent patient data and documentation in medical record in accordance with state, TJC and Mercyhealth system guidelines.
Attends required meetings or staffing as indicated by Program Manager.
Participates in performance improvement activities.
Performs other duties as assigned.
Complies with all Mercy Health System, State and JCAHO policies and regulations.
EDUCATION AND EXPERIENCE
Master's or Doctorate degree in counseling, Social Work, Psychology or other related field
CERTIFICATION AND LICENSURE
Must possess all required certification and licensure per the state in which they will be practicing.
Wisconsin: LCSW, LPC, or LMFT
Illinois: LCSW, LCPC, LMFT
Open to those with Dual Licensure:
Wisconsin: Wisconsin Substance Abuse Counselor--SAC or Clinical Substance; Abuse Counselor--CSAC
Illinois: CADC or CRADC; TACT Training Level 2; CPR/BLS preferred
ADDITIONAL REQUIREMENTS
Passing the Driver’s License Check and or Credit Check (for those positions requiring).
Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.
Must be able to follow written/oral instructions.

The noise level in the work environment is usually quiet

Patients, families, physicians, community and regulatory agencies, and other hospital and clinic personnel.

LEVEL OF SUPERVISION
May supervise student interns as assigned. Requires a high level of independent judgment. The position has the potential for substantial latitude and independent action.
SUPERVISES
Supervised by the Clinical Supervisor, Psychiatry staff members and/or Program Supervisor Manager.
PAY RANGE:
