Pay range: $55-$60/hour, depending on experience.
Responsibilities:
- Assess patients’ physical, psychological, social, cultural, and spiritual needs.
- Develop individualized care plans focused on symptom management and quality of life.
- Coordinate care with interdisciplinary team members for comprehensive support.
- Educate patients and families on hospice philosophy and end-of-life care.
- Monitor patient progress, adjusting care plans as needed.
- Facilitate seamless communication to ensure continuity of care.
- Conduct follow-ups to maintain care compliance and resolve concerns.
- Identify and address barriers to successful care transitions.
- Advocate for patient needs from pre-admission through discharge.
- Collaborate with clinical teams to source hospice referrals.
- Review Explanation of Benefits (EOBs) and consent documents with patients.
- Perform nursing assessments to evaluate hospice eligibility.
- Admit qualifying patients for general inpatient hospice care.
- Support hospice service line through sales, marketing, and public relations efforts.
Qualifications:
- Nursing degree from an accredited school; bachelor's preferred.
- Minimum of 1year of hospice care experience, plus 1+ years in acute and home care settings.
- Active state nursing license and BLS CPR certification.
- Supervises hospice aides; prior management experience not required.
- Strong assessment, communication, problem-solving, and nursing skills.
- Ability to lift, transfer, and position patients; frequent movement required.
- Valid driver’s license with insured, reliable transportation.
- Preferred experience in holistic or alternative care modalities.
