Job Description Summary

The Certified Nursing Assistant is an integral part of the Personal Assistance Services team. Under the direction of the Director of Client Services, the Certified Nursing Assistant must show proof of successful completion of training and/or demonstrated competency in and knowledge of:

  • Responsibilities and limitations communication techniques
  • Client rights confidentiality of PHI
  • Respect for person and property
  • Observation for client changes to report
  • Standard infection control
  • Home safety precautions
  • Basic body functions and variances
  • Procedures for clean, safe environment
  • Emergency response
  • Physical, emotional, and mental client care
  • Bed, sponge, tub or shower bath
  • Skin care, oral hygiene, toileting
  • Basic nutrition, meal preparation
  • Encouraging independent care
  • Documentation techniques
  • Supervisory support and duties
  • Advance directives implications

Responsibilities of the Certified Nursing Assistant include the following:

  1. Demonstrates support for the agency mission statement to promote quality, efficient, comprehensive, and effective client services.
  2. Participates in and supports the agency’s performance improvement activities.
  3. Provides Certified Nursing Assistant services to clients as identified by the agency in the plan of services per agency policies, procedures, and Certified Nursing Assistant skill level.
  4. A “mandated reporter” of suspected abuse of dependent adults.
  5. Prepares and maintains accurate and timely client activity notes.
  6. Assists with personal care activities (ADLs) and IADLs as indicated on the care plan.

Including:

  1. Personal care and hygiene
  2. Universal precautions
  3. Locomotion/ambulation/fall prevention oversight
  4. Nutritional support, meal preparation, feeding assistance
  5. Bed mobility and positioning, range of motion exercises
  6. Toileting
  7. Continence care
  8. Dressing and undressing
  9. Bathing
  10. Foot care (nails filed only)
  11. Skin care
  12. Medication reminder assistance
  13. Light housekeeping including dusting, vacuuming, linen changes, cleaning the kitchen and bathroom
  14. Laundry of client’s clothing and bedding.
  15. Social and emotional support
  16. Reports client condition changes to supervision.
  17. Appropriately seeks supervisory assistance as necessary to effectively perform duties.
  18. Maintains confidentiality of client, employee and agency operations.
  19. Participates in 5 hours of annual in-service training as assigned.
  20. Responsible for proper use and care of equipment.
  21. Practices safety, universal precautions, and infection control measures.
  22. Maintains a neat appearance and positive attitude with clients, family, and personnel.
  23. Performs other duties as qualified and requested.

Minimum Eligibility Requirements

  • Must be at least 18 years of age and be registered/or agree to be registered with the State of Texas Health Care Services Bureau Registry.
  • Must possess a valid Certified Nursing Assistant license in the state of Texas
  • Prefer a high school degree or GED.
  • Must have at least one year of employment experiences as a Certified Nursing Assistant
  • Excellent observation, verbal and written communication skills, problem solving skills
  • Valid driver's license and proof of insurance is required
  • Background check and drug clearance

improve performance.

Physical/Sensory Requirements (with or without the aid of mechanical devices)

Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient and family individualized psychosocial needs. Requires eye-hand coordination and manual dexterity.

Environmental and Working Conditions:

Works in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather
