CNA/LPN 1:1 Direct Support Position

Position Title: CNA or LPN – 1:1 In-Home Support Staff
Location: Group Home Setting with one person 
Schedule: Full-Time and Part-Time Opportunities Available | 24/7 Coverage
Reports To: Designated Coordinator/Designated Manager

Position Summary

We are seeking a compassionate, dependable, and patient-focused CNA or LPN to provide dedicated 1:1 support services for an individual living independently in their own home. This position plays a vital role in supporting the person served with daily health and personal care needs while encouraging independence, comfort, safety, and community involvement.

The ideal candidate will enjoy building meaningful relationships and assisting someone who loves coloring, shopping trips, and relaxing outdoors to enjoy the weather.

Essential Responsibilities

Personal Care & Daily Living Support

• Assist with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility support


• Maintain a clean, safe, and organized home environment

Medical & Health Support

• Administer medications according to physician orders and company policies


• Provide tube feeding care and monitoring


• Observe and document changes in health status, behaviors, or medical concerns


• Monitor for signs of illness, discomfort, or medical emergencies


• Communicate effectively with healthcare providers, guardians, nursing staff, and supervisors

Community & Recreational Support

• Accompany and support the person served during shopping outings and community activities


• Encourage participation in preferred hobbies and activities such as coloring and spending time outdoors


• Promote independence, choice-making, and quality of life

Documentation & Compliance

• Complete accurate daily documentation and charting


• Follow individualized support plans, medical protocols, and MN 245D policies/procedures


• Maintain confidentiality and professional boundaries at all times


• Attend required trainings and staff meetings

Qualifications

• Current CNA certification or active LPN license


• Valid driver’s license and acceptable driving record


• Ability to pass background study requirements


• Experience working with individuals requiring high levels of medical and personal care preferred


• Experience with tube feeding and medication administration preferred


• Compassionate, reliable, patient, and self-motivated

Physical Requirements

• Ability to stand, walk, bend, and assist throughout shifts


• Ability to respond appropriately during emergencies
