Responsibilities:
Under the supervision of the Program Director, provide direct case management services to program participants, including assessing needs, developing and implementing individualized service plans, and providing ongoing support and guidance.
Conduct needs assessments, develop case plans, and provide crisis intervention services as needed.
Facilitate access to community resources and services, including housing, healthcare, education, and employment opportunities.
Evaluate efficiency and effectiveness of community service providers on an ongoing basis to ensure residents are receiving quality care.
Ensure compliance with grant requirements and regulations, including documentation and reporting.
Track and document resident progress, outcomes, and service delivery.
Collaborate with stakeholders and community partners to ensure effective service delivery and achieve program goals.
Foster a supportive and inclusive environment, promoting resident engagement and empowerment.
Communicate effectively with residents, staff, and community partners, ensuring seamless service delivery and coordination.
Collaborate with service providers to ensure comprehensive support for residents.
Initiate/attend partner meetings to case conference specific resident needs, identifying referral options and program service gaps. Work closely with local agencies, organizations, and the community to ensure understanding of specific resident needs, identifying referral options and program service gaps.
Identify, refer, and regularly check in on hand-offs made to community partners.
Conduct needs assessments of residents’ needs, goals, and eligibility for services.
Develop and implement individualized service plans, ensuring alignment with grant objectives and resident needs.
Provide immediate support and crisis intervention services to residents, addressing urgent needs and stabilizing structure.
Link residents to community resources, services, and support networks, facilitating access to essential services.
Track resident progress, outcomes, and service delivery, ensuring grant requirements are met and client needs are addressed.
Advocate on behalf of residents, navigating complex systems and overcoming barriers to access essential services.
Collect and track data on resident participation outcomes, service delivery, and program effectiveness.
Prepare and submit reports as required by the grant, ensuring timely and accurate documentation.
Other duties as assigned.

Education: Bachelor’s degree in social work or related field preferred; any appropriate combination of education, experience and/or certification will be considered.
Experience: A minimum of 2 years’ experience in the same or similar job role is preferred.
Language: Proficiency in oral/written English; Spanish preferred; additional languages a plus (i.e. Creole)
Skills: Proficiency in computer (Microsoft Office and other data collecting programs)
Successful completion of toxicology test and of Level 2 background screening, and OIG.
Have a Valid FL Driver License with clean driving record and be legally authorized to work in the U.S. on a full-time basis.
Culturally sensitive to the social, psychological and healthcare needs of the vulnerable populations serviced.
Strong organizational, interpersonal and communication skills in order to meet deadlines and handle multiple tasks.
Demonstrates a high level of productivity while remaining calm and effective under pressure.
A self-starter with demonstrated ability to work independently, as well as, with multidisciplinary teams and co-workers.
Ability to maintain a high level of poise and professionalism in all circumstances.
Ability to establish supportive, respectful, trusting relationships with persons with histories of homelessness, severe and persistent mental illnesses, trauma, and co-occurring substance use disorders.
