Job Description - Co-Director CRNP & PA-C Services

Description

 POSITION SUMMARY
A Co-Director of CRNP & PA-C Services provides primary care services, including assessing and managing patients in the clinic, using the patient centered medical home concept, and the following established standards and practices of the clinic. The position combines the roles of provider, mentor, educator, researcher and administrator. The PA must honor the Commonwealth of Pennsylvania’s Physician Assistant guidelines and ethical standards. The PA must have and exercise the ability to communicate effectively, both verbally and in writing, with advanced interpersonal communication skills, and must at all times comply with applicable standards and exercise appropriate care to protect confidentiality, privacy and security of patient protected health information. The employee must perform all other duties as assigned.

ESSENTIAL JOB DUTIES AND FUNCTIONS

While living and demonstrating our Core Values, the Co-Director of CRNP & PA-C Services shall perform the following essential job duties and functions:

  • Obtain, compile and record patient medical data, including health history, progress notes and results of physical examination
  • Interpret and integrate data to determine appropriate treatment plans as needed
  • Instruct and counsel patients. Communicate clearly and effectively with patients and other health-care workers
  • Collaborate with physicians in managing acute and long-term medical needs of patients
  • Order necessary diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests
  • Interpret diagnostic test results including to assess and identify deviations from normal 
  • Recommend resources to meet patient and family needs
  • Provide counseling and instruction regarding patient treatment and follow-up care
  • Provide effective and meaningful patient education
  • Effectively identify, evaluate, and address disease prevention and health promotion issues of the population in the practice while administering quality patient care
  • Understand and participate in meaningful use data
  • Utilize and document in the electronic health record
  • Administer emergency care per ACLS or PALS standards
  • Document medical records with all patient encounters in the office and on the telephone while staying compliant and up-to-date with the EMR process
  • Prescribe therapy or medication, with physician approval as appropriate
  • Perform therapeutic procedures, such as injections (excluding hip joint injections/aspirations), immunizations, suturing and minor/superficial wound care, and infection management
  • Exercise HIPAA confidentiality and security measures at all times during office hours and outside of the office
  • Demonstrate responsibility for self-learning through participation in continuing education activities and conferences
  • Participate in quality improvement reviews and activities in the clinical setting
  • Provide physicians with assistance during medical procedures as needed
  • Complete documents, charts, triages etc., in accordance with the organization's open document policy
  • Work with PA students and other students that train at TWCCH 
  • Practice as an outpatient provider with thirty-four (34) hours of patient care per week (with availability for 3 patient slots per hour, 102 patient slots per week) and six (6) hours of administrative time per week, and must be present on-site at least forty (40) hours per week, which includes administrative time. Administrative time must be coordinated and planned to occur outside of patient care hours and when there is otherwise an adequate number of providers to cover overall patient care responsibilities.  The particular days scheduled will be worked out with the Medical Director and Practice Manager
  • Must have zero charts overdue > 10 days no more than 100 at end of month
  • Monitor lab and document box of other providers as assigned and direct any questions or concerns to the Medical Director
  • Work some late nights, on average one to two nights per week
  • Participate in the outpatient call schedule, which typically includes one weekend per month and the occasional weekday as needed, with physician as backup.
  • Schedule will include participation in weekend rotation including up to two Sundays per month, or two Saturdays per month, or any combination of one Saturday and one Sunday per month, at any of Corporation’s practice locations; however, additional weekend rotation days could be scheduled from time to time based on operational and clinical needs of the Corporation as determined by Corporation in its sole discretion
  • Participate in holiday rotation, including to work 1-2 holidays per year (with use of a floating day off within 90 days)
  • Work additional hours/days to cover colleague time off and staffing emergencies as needed
  • Timely completion of Peer Reviews as assigned by Credentialing department to be compliance standards
  • Perform other duties as assigned

Medication Administration

  • Administration of medications and injections, including Tetanus prophylaxis and non-prescription medicine
  • Prescribe and dispense relevant to the area of practice for the PA
  • Prescribe and dispense schedule II with DEA certificate
  • Prescribe and dispense schedule III, IV and V with DEA certificate

Basic Invasive Procedures

  • Ability to do the following
    • Cautery
    • Epistaxis management
    • Urethral catheter insertion and removal
    • Collection and preparation of specimens, i.e. gram strains, C&S
    • EKG tracings
    • Pap Smears
    • Foreign body removal: ears, nose, throat, rectum, soft tissue

Ophthalmologic Procedures

  • Ability to do the following:
    • Visual screening
    • Corneal screening
    • Simple removal of foreign bodies
    • Conjunctiva
    • Visual Activity

Surgical Procedures

  • Ability to do the following
    • Subcutaneous local and digital anesthesia
    • Repair simple lacerations
    • Laceration management
    • Cleansing/irrigation
    • Simple edge revisions/suturing
    • Suture and staple removal
    • Treatment for minor superficial (1st and 2nd degree) burns
    • Incision and drainage of abscess
    • Wound irrigation, packing
    • Control of external hemorrhage
    • Carry out aseptic and isolation techniques

Care Coordination

  • Maintain ongoing appropriate documentation on care coordination to promote awareness and ensure patient safety and follow-through on care plan.
  • Assist patients in problem solving potential issues related to the health care system, financial and psychological barriers.
  • Functioning as the system navigator and point-of-contact for high-risk patients and family, with the patient and family having direct access for asking questions and raising concerns.

SUPERVISION:

With respect to the duties and responsibilities of the Physician Assistant as described in this Position Description:  

  • The duties and tasks will be performed in an outpatient office
  • Daily onsite direct and indirect supervision will be provided to the Physician Assistant at the outpatient office; the supervising physician will be at a minimum available to the Physician Assistant via telecommunication; if the supervising physician is not available to the Physician Assistant, a back-up supervising physician, the substitute Physician Assistant supervisor will be identified for the consultation and direction of the PA’s activities
  • The supervising physician or substitute physician will see patients at the request of the PA, the patient, and/or the patient’s family
  • The supervising physician or the substitute physician will review and sign 100% of the medical records of the patients cared for by the PA within 10 days

DIRECTOR ROLE & RESPONSIBILITIES:

  • Develop and maintain schedule for all PA and CRNPs in clinical settings to include on a call and holiday schedules.
  • Participate in the recruitment process for all PA and CRNP new hires.
  • Lead the orientation and training of new PA-C/CRNPs. 
    • Create a comprehensive orientation checklist for clinical as well as administrative responsibilities. 
  • Staff Meetings
    • Regularly attend clinic huddles and arrange for coverage when needed.
    • Participate in creating agenda and provide update at meeting.
  • Performance Management
    • Conduct performance evaluation of PAs and CRNPs to include probationary and annual performance reviews, and implement performance improvement plans as needed.
    • Ensure PA/CRNP engagement in organizational performance standards PACE Card metric, PDSAs, SAFE reporting, and community events. 
    • Conduct root cause analysis of patient satisfaction survey results. 
    • Demonstrate organizational pride with highest integrity to create a high performing clinical team and create an atmosphere that encourages teamwork and collaboration.  
    • Work with management team to proactively address issues/concerns and collaborate to resolve in a positive and professional manner. 
  • Compliance and Supervision of TWC standards of care
    • Ensure compliance with clinical policies and procedures to include but not limited to charting, triages, labs.   Ensure compliance with Open Document policy and other policies.
  • Medical Group Board Participation
    • Attend and actively participate in monthly Medical Group Board meeting, adjust schedules as necessary
  • Responsible for all clinical and administrative needs of all PAs and CRNPs across the organization 

Requirements

 REQUIRED QUALIFICATIONS

  • Meet The Wright Center for Community Health and its affiliated entity The Wright Center for Graduate Medical Education EOS© People Analyzer Tool
  • Buy in and experience working in the EOS® model (strongly preferred)
  • Mission-oriented; represents the enterprise in a professional manner while demonstrating organizational pride
  • Physician Assistant Degree from an accredited educational program for physician assistants
  • Certification by NCCPA 
  • Licensed Physician Assistant in the State of Pennsylvania 
  • Valid DEA Registration Certificate
  • Demonstrated expertise in relevant medical practices, protocol, trends and best practices in clinical areas assigned. Demonstrated knowledge and success in affecting overall clinical operations 
  • Ability to manage multiple responsibilities and emergency situations successfully.  
  • Proficiency in knowledge of office management, use of computer, software, and office machines
  • Familiarity with electronic health records, preferably Medent
  • Current BLS, ACLS and PALS certified
  • Valid PA driver’s license
