The Opportunity



We are seeking a Community Companion Home (CCH) Coordinator to provide leadership, oversight, and hands-on support to Home Care Providers, ensuring the health, safety, and overall well-being of individuals in Shared Living arrangements.


This role involves a high level of travel throughout the state of Connecticut—visiting homes, meeting with providers, and connecting with individuals in their communities. Monthly visits in Connecticut are required for each provider, and the Coordinator will also assist with the initial licensing process for potential providers. The CCH Coordinator builds strong, trusting relationships with providers and the individuals they support, ensuring each person has opportunities for personal growth, meaningful community connections, and a high quality of life. You will collaborate with Aspire’s management team, state agencies, family members, medical professionals, and other stakeholders to create supportive, inclusive environments where individuals can flourish.



 


What You’ll Do



  • Support and oversee Home Care Providers to ensure individuals’ safety and needs are met.

  • Conduct monthly visits and provide guidance to Home Care Providers.

  • Collaborate with Aspire’s team and community partners to support individuals’ growth and well-being.

  • Travel extensively across Connecticut to meet with providers and individuals.


 


Your Schedule
This is a full-time position which is typically a Monday–Friday schedule, 9:00 a.m. to 5:00 p.m. Flexibility for occasional evenings and weekends is required to meet the needs of providers, especially for monthly visits.


Salary: $58,000/YR


 


What You Bring



  • Bachelor’s degree or equivalent professional experience.

  • Prior experience supporting individuals with disabilities is strongly preferred.

  • Reliable transportation, a valid driver’s license, and willingness to travel.

  • Strong communication skills, empathy, and enthusiasm for making a difference.

  • Basic computer skills (experience with Therap is a plus!).
