Capstone Hospice is in search of a Community Enrichment Coordinator to assist in strengthening professional relationships within senior living communities.   This position is almost exclusively field-based an involves regular visitation to targeted assisted living communities, personal care homes, and nursing homes.  This position will meet professional goals by providing value-added activities to an assigned number of accounts.

Essential Functions of the position: 

  

1. Partners with Community Education Liaisons to build and maintain relationships with senior living communities that result in admissions to Capstone Hospice.

2. Maintains visible presence in targeted accounts, mainly through resident activities.

3. Works with the Activities Director in targeted accounts each quarter to establish a regular presence in the building through assisting with or coordinating activities for the building’s residents.

4. Coordinates or assists with 25-30 activities per month – min 1 per month per each targeted building.

5. Works with assigned Community Education Liaison to organize and schedule Quarterly Partnership Meetings with identified buildings.

6. Attends weekly sales meeting.

7. Reviews and completes hospice Consent Forms with patients and families when requested. 

8. Submits weekly schedule and weekly review of activity from the previous week. 

9. Submits updates (both verbal and written) as requested (daily, weekly, and/or quarterly)

10. Adheres to marketing budget guidelines as established by management

11. Become a hospice education resource by conducting in-services in conjunction with clinical staff that promote Capstone Hospice services to senior living communities.

12. Meets with patients or their family members within care facilities or private residences

13. Ensures compliance with state and federal regulations

14. Reports activities using required technology daily (email, Outlook, CRM, text, etc.) 

12. Is always ready to share a positioning statement that explains the unique of working with Capstone Hospice

13. Attends staff meetings, IDG and other meetings as assigned and appropriate.

14. Participates in the orientation program as assigned.

15. Adheres to all Hospice policies.

16. Assumes responsibility for own personal and professional development and maintenance of skills in sales and marketing.

17. Exhibits hospice philosophy in all job-related roles.

18. Other duties as assigned by Director of Business Development.

This position is supervised by the Director of Business Development

This position has no direct reports. 

Interrelationships with patients, families, referral sources (especially senior living communities), IDG and other healthcare team members. 

Requirements

Candidates must meet the following requirements to be considered: 

  • Bachelor's degree from an accredited institution, OR 
  • Associates degree with minimum two years of healthcare or sales experience. 
  • 2 years of healthcare and/or sales experience.

Candidates should possess the following knowledge, skills and abilities to be successful in the role: 

  • Knowlege of proven account management and sales call techniques, preferred.
  • Ability to work independently, make accurate, and at times, quick judgements. 
  • Strong customer service and follow-up skills. 
  • Excellent presentation, verbal and written communication skills. 
  • Highly ethical, self-motivated team player. 
  • Acceptance of an adaptability to different social, racial, cultural, and religious settings. 

  

Mental Requirements:

· Must be able to work independently, make judgments based on assessments and data available and act accordingly.

· Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

Working Conditions:

· Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.

Transportation:

· Must have a current valid driver's license, auto liability insurance and reliable transportation.

This position is a full-time salaried exempt position with bonus potential.
