PUYALLUP TRIBAL HEALTH AUTHORITY, located in Tacoma, Washington is seeking a Community Health Manager. In alignment with PTHA goals, responsibilities include developing and leading community health programs/initiatives, securing funds for programs/initiatives, and coordinating and overseeing all community health activities for the organization to ensure the delivery of quality health care services.

ESSENTIAL FUNCTIONS:

• Manage the daily operations of the Community Health Department.
• Work closely with Clinical team in the development and management of programs and procedures to accomplish community health goals and policies.
• Serve as the primary contact for PTHA related to healthcare services being provided at Chief Leschi Schools, House of Respect, Tribal Jail and other community and Tribal locations.
• Manage the active engagement of community health program implementation, budget development, as well as program evaluation.
• Develop measures of success and tracks the impact of health programming on the community. Identifies community needs assessments and develops measurable standards of performance to improve health outcomes in community.
• Implement and manage community health programs. Including related grant management.
• Build and maintain strong relationships with Tribal entities, and other key stakeholders to promote and expand outreach and engagement efforts to ensure success of community health initiatives.
• Provide and coordinate after-hours on-call duties.
• Plan, monitor and execute staffing requirements including recruitment, retention, training, management, motivation, supervision, conducting performance evaluations/competency assessments, implementation of training plans and disciplinary actions up to termination.
• Review professional staff caseload management and provide clinical consultation as appropriate.
• Develop and monitor annual budget for the Community Health department.
• Participate in special projects for the department and PTHA.
• Provide administrative back-up for the Professional Services Director in their absence.
• Responsible for community health quality improvement efforts, reports, committee representation, utilization review.
• Participate in the development and implementation of goals, objectives, and policies and procedures for the PTHA.
• Address patient and department complaints.
• Provide leadership intra and interdepartmentally to develop solutions, and address patient and employee complaints and conflicts.
• Responsible for coordination with Tribal programs to ensure quality care to Tribal Members. Work in collaboration with PTHA leadership to establish and update working agreements for offsite locations.
• Responsible for ongoing compliance monitoring, training, and staffing for off-site services (Restorative Justice Center, Tribal Jail, House of Respect, Chief Leschi Schools, NFP).
• Provide direct nursing care to patients as needed.

Requirements

• Bachelor of Science Nursing (BSN) Degree.
• Current WA State RN license.
• Five (5) years’ experience in nursing. 
• Minimum of three (3) years’ experience directly supervising employees. 
• Minimum two (2) years’ experience with program planning, development and continued quality improvement. 
• Must have significant experience building and maintaining internal and external relationships and influencing key constituents and decision makers. 
• Must have excellent written, verbal, presentation and interpersonal skills.
• Certification in ACLS/PALS, or ability to obtain certification within six (6) months of hire.
• Experience working with an electronic health records system within the last year.
