Community Health Worker

The Rockbridge Area Health Center is seeking a full-time Community Health Worker to join our team.

The Community Health Workers (CHW) will primarily be working with specific target populations, community partners, primary care teams, and other agencies to improve access, patient care, and outcomes because of their deep understanding of the cultural norms of the people they serve, as they are often members of the community themselves. CHWs find and meet people where they are in life and assist them in improving their life and health. CHWs focus on four essential areas: Relate, Educate, Navigate, and Advocate.

• Education, Credentials, and Experience
  
  
  
  • High school diploma or equivalent required.
  
  
  • Any combination of 3 years health/social services experience and/or technical degree, associate in arts degree, or bachelor’s degree in a human service-related field preferred.
  
  
  • CHW certification or willingness to obtain certification within 18 months of hire and maintain certification.
  
  
  • Certified Application Counselor (CAC) certification or willingness to obtain certification within 18 months of hire and maintain certification.
  
  
  • Current Basic Life Support (BLS) certification with hands on practicum required.
  
  
  • Valid driver’s license and reliable transportation required.

  • Individual must be professional, self-motivated and have strong attention to detail, communication, teamwork, customer service and computer skills.
