Type: Full-Time: 40 hrs

Salary: $18.31-$18.91/hr DOE

Key Responsibilities:

• Client Assistance: Assists patients in the clinic setting to provide support and guidance on various resources and benefits. 


• Program Communication: Explain the benefits of the Community Health program to clients/patients and how it can improve their wellbeing.


• Health Management: Help patients identify socio-economic issues affecting their health and develop management plans.


• Documentation: Maintain comprehensive electronic client files, documenting encounters, referrals, and outcomes.


• Service Coordination: Facilitate communication and coordinate services between providers and clients to ensure effective care.

What We’re Looking For:

• Proficient computer skills, including internet-based applications


• Ability to thrive in a fast-paced environment while managing multiple priorities


• Strong organizational skills and accountability for time and activities with remote supervision


• A passion for serving others with professionalism and clear boundaries


• Community involvement or membership in the community served

Requirements: 

• High School Diploma or GED


• Valid Driver's License
  

Additional Certifications/Trainings upon hire: 

• BLS Certificate 


• Community Health Worker Certificate within the first six months of employment


• Certified Application's Counselor (CAC) within one year of employment
