Community Health Worker

Are you someone who looks out for others?  Helps neighbors? Our Y is looking for people with a gift for caring to join our Community Health Worker team. These individuals help navigate and coordinate community, healthcare and social service supports for vulnerable populations. This position will utilize the evidence -based Pathways Community Cares model in partnership with the Great Rivers United Way to accomplish this goal. You will join a supportive team in making a positive impact on the lives of La Crosse area families. 

Responsibilities:

• Assist clients in accessing community resources, including health insurance, food, housing, quality medical care and employment.
  

• Partner with local schools to help families improve attendance 
  

• Facilitate communication and client empowerment in interactions with health care/social service systems.
  

• ​Document client interactions and completed work on a computer database.
  

Requirements

• High School Diploma
  

• Experience working with families with young children
  

• Lived experience, case management or social service experience a plus
  

• Bilingual a plus
