Description
Under direct supervision, responsible for assisting individuals and communities with adopting healthy behaviors by establishing trusting relationships with patients and their families while providing general support and encouragement. Promotes and facilitates use of health care and social service systems.

Responsibilities

Bilingual preferred
Outreach calls to surgical patients before their procedure to check in and remind them of appointments, testing, and next steps
Screening for barriers that could delay or cancel surgery, such as transportation, a ride home, caregiver support, or difficulty understanding instructions
Connecting patients to community resources, social services, and their primary care provider
Reinforcing pre-operative instructions from the surgeon, anesthesia, and nursing, and routing any clinical or medication questions to the nurse or provider right away
Helping patients and families plan for recovery at home and connecting them to the right team for equipment, home health, or follow-up
Sharing patient needs with the PAT team in huddles and documenting in Epic

Qualifications

Demonstrated knowledge of health and social services systems and familiarity with community resources and services.
Ability to maintain full confidentiality of program participant information and data.
Ability to shape others' understanding in ways that capture interest, informs, and encourages participation.
Builds and maintains positive working relationships, with individuals and work groups through open communication and collaboration; works with others to accomplish goals and objectives.
Proficiency in Microsoft Office Suite.
