Job Description:
Determine the individual’s needs and screen for potential eligibility for services, utilizing a standardized screening instrument.
Provide efficient, timely, and consumer-friendly services to facilitate the eligibility review.
Provide the necessary forms and package them in preparation for service determination.
Assist the individual in collecting, completing, and gathering required information and documents for the services. This assistance may be provided in the individual’s home. The activity involves assisting potential applicants as a secondary resource to family members and care providers.
Coordinate with staff to identify potentially eligible individuals and monitor their needs.
Help support adherence to the treatment plan and monitoring client needs
Help patients develop health and management plans and goals, including creating a Plan of Care (POC) tailored to their specific needs and incorporating feedback from their family.
Assist in obtaining the Physician Referral form for probable individuals and coordinate with health insurance, community partners, and the team.
Comply with follow-up and reassessment policies to monitor client status.
Act as a client advocate in a complex service delivery system, maintaining an open line of communication to ensure good customer relations and monitor system efficiency and protocols for at-risk clients and their caregivers.
Maintains customer and employee confidence and protects client confidentiality.
Helping support adherence to the treatment plan, monitoring the patient’s needs daily.
Prepares reports by collecting and analyzing information.
Secures information by completing database backups.
Maintains professional and technical knowledge by attending educational workshops and reviewing professional publications.
Other duties as assigned
Qualifications
Have an intake specialist, nurse, or related work experience with the long-term care client population, preferably Medicaid-related experience; or,
Haverelated work experience with the long-term care client population, preferably Medicaid-related expertise, or a Community Health Worker certificate
LPN, RN, or Bachelor's Degree preferred
Knowledge in the following areas is required: People Skills, Marketing, Branding, Public Speaking, Fostering Teamwork, Planning, Building Relationships, Initiative, Customer Focus, and Oral and written communication.
A high level of personal skills is required to formally present to groups and deal effectively with people from all community segments.
The individual must be comfortable with diversity and respectful of a wide range of faiths, beliefs, and experiences.
