The Community Health Worker (CHW) serves as a trusted link between the health department and the community by building strong relationships through consistent outreach and engagement. This position focuses on improving health outcomes by providing culturally appropriate health education on topics such as chronic disease prevention and management, maternal and child health, immunizations, infectious disease prevention, nutrition, mental health, and healthy lifestyles. The CHW assists individuals and families in navigating healthcare systems, social services, and community resources, including insurance enrollment, transportation, housing, food assistance, childcare, and financial support programs. Outreach is conducted in a variety of community settings such as homes, schools, workplaces, faith-based organizations, and community events. The CHW identifies barriers to care, advocates for client needs, and empowers individuals to build self-sufficiency while supporting care coordination through communication with healthcare providers, public health staff, and partner agencies. Responsibilities also include collecting and documenting client interactions, referrals, and outcomes in accordance with program requirements, confidentiality standards, and data reporting expectations; participating in community assessments, program planning, and evaluation activities; and assisting with public health initiatives such as vaccination clinics, health screenings, emergency response activities, and health promotion campaigns. The CHW maintains up-to-date knowledge of local resources and referral processes, represents the health department professionally in the community, and contributes to a positive and collaborative team culture. Strong interpersonal and communication skills, the ability to work independently in a flexible, field-based role, basic computer and documentation skills, a commitment to confidentiality, problem-solving ability, and willingness to travel throughout the service area—including rural and remote locations—are required.
