Job Description:

Job Summary

Responsible for serving as a client advocate and liaison to support patient engagement and research participation. Provides care coordination and health education to assist patients in managing chronic disease and achieving healthier lifestyles. Assesses patient needs, makes referral recommendations, and facilitates access to health and social services. Advocates on behalf of patients and families with service providers and agencies. Conducts home visits and maintains one-on-one interactions to deliver personalized support to at-risk clients.

Essential Functions

• Assesses at-risk clients’ needs and prioritizes care coordination and service delivery.

• Conducts home visits and provides health coaching on chronic conditions and smoking cessation.

• Advocates for clients and facilitates referrals to health and human services agencies.

• Coordinates and monitors self-management plans to support clients in achieving better health outcomes.

• Assesses eligibility, enrolls, and monitors children and families in health coverage programs such as CHIP and Medicaid.

• Recruits and enrolls clients in research studies, administers study measures, and ensures compliance with IRB protocols.

• Maintains accurate, complete, and confidential documentation and data entry in required databases.

• Networks with resource agencies and multidisciplinary teams to connect clients to needed services and participates in ongoing training and education.

• Performs other duties as assigned.

Education Requirement

High school diploma or equivalent experience

Experience Requirement

0-1 years of experience in a healthcare, clinical setting, or a relevant area

An equivalent combination of education and experience may be considered.

All experience must be paid and in the same related field.

Part-time and PRN experience will be prorated based on hours worked per week.

Volunteer work and internships for academic credit are not counted.

Physical Demands
