We believe in the power of community support, and we're looking for compassionate individuals to join us as Community Health Workers. In this role, you'll play a crucial part in assisting patients in our community. Your responsibilities will include:

Overview

The Infant Maternal Health Community Health Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up. visits for patient wellbeing.

Community Health Specialists supporting the Infant and Maternal Health Program will spend 5 days on-site in a designated clinic/office supporting various workflows, including patient outreach to help improve family’s barriers, and resources. Including patient outreach to help improve childhood immunization vaccination rates. The Community Health Specialists supporting the Infant and Maternal Health Program will support various workflows, including patient outreach to help address numerous barriers that families encounter, and connecting them to available resources on-site in a designated clinic/office. Patient outreach will also include efforts to help improve childhood immunization vaccination rates. You will be required to participate in team meetings/retreats, which may be in-person. Training and education will be provided as part of the orientation process.

Community Health Specialist must have reliable transportation to conduct home visits, with mileage reimbursement. Community Health Specialists are also required to be available for community events that take place during non-traditional work hours (up to 10% of time).

Requirements:

• High School Diploma or equivalent required.
• 0-3 years of experience required.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.
