Qualifications: 

• High school diploma or equivalent. 


• At least two (2) years of experience working in a healthcare or clinical setting in the support of persons served. 


• Must possess a valid driver's license in the State of Michigan and proof of insurance. 


• Ability to work closely and communicate effectively with persons with mental and/or physical limitations. 


• Must possess a knowledge and understanding of co-occurring disorders (mental health & substance abuse). 

Duties and Responsibilities:

1. Establishes trusting relationships with persons served as they work collaboratively and effectively with clinical care teams. 


2. Provides ongoing interviewing, goal setting and follow-up for persons served in the community. 


3. Effectively communications Easterseals MORC's mission i establishing relationships with medical staff and persons served. 


4. Acts in advocacy and liaison role between persons served and community service agencies. 


5. Assists persons served in overcoming barriers to obtaining needed medical care and/or behavioral health/social services. 


6. Connects persons served with transportation resources when appropriate. 


7. Assists with the completion of required information such as enrollment, eligibility determination and overall follow-up. Assist in the completion of applications and registration forms when needed in order to expedite care. 


8. Records and tracks care management information. Assesses information to identify patterns and/or trends. 


9. Maintains knowledge of community resources to assist in meeting the needs of persons served. 


10. Provides referrals for community services as appropriate. 


11. Assists persons served in the development of health management plans and personal care goals. Provides coaching in the effective management of their chronic health conditions and self-care. Motivates individuals to be active and engaged in their health care.
