What You’ll Do:
As a Community Health Worker focused on behavioral health, you’ll work closely with patients facing mental health and substance use challenges, including those in Medication-Assisted Treatment (MAT). You’ll help them find their way through the healthcare system, connect to resources, and support them in reaching their goals. Your work will help make healthcare more accessible and equitable in our community.
Day-to-Day Responsibilities:
Build trust and strong relationships with patients.
Help patients find and use community services and healthcare programs.
Team up with our medical, behavioral health, and case management staff to make sure patients get well-rounded support.
Do outreach in the community to connect with people who might not be getting care.
Support patients with setting goals and staying on track with their care.
Provide education on mental health, substance use, managing chronic conditions, and prevention.
Refer patients to services like housing, food, transportation, and behavioral health programs.
Keep accurate notes in the Electronic Medical Record (EMR) system.

Requirements
High school diploma or equivalent.
Great people skills and ability to connect with others.
Good communication and organization.
Comfortable working with people from all kinds of backgrounds.
Passionate about helping others break down barriers to care.
Have and maintain a valid California Driver’s License. Must carry liability/bodily injury on his/her automobile(s) used for work
Preferred:
Bilingual in English and Spanish.
Associate’s degree in health, behavioral health, or social services-related field
Completion of a Community Health Worker training program
Experience or education related to Community Health Worker/Promatora, Health or Human Services, health promotion or other health related field.
Experience in behavioral health, case management, community outreach and working with underserved populations
Familiarity with EMR systems and local community resources
