Minimum Qualification
- Must be 18 years of age.
- Possess a valid Driver’s license.
- Person in this position must be able to move about in the community 80% of the time.
- Must have valid CPR/First Aid Card. (Provided by Oak and Cedar).
- Must receive clearance to work by the Department of Justice (Live scan).
- Must have a have reliable transportation
Duties and Responsibilities
- Willingness to transport participants in personal vehicle.
- Work at a 1:2 staff to client ratio
- Promote Person Centered Thinking, Client Choice, Independence, etc.
- Provide support in the following areas: Recreation, Socialization, Health and Safety,
- Community Inclusion, Communication, and Self-Advocacy.
- Maintain schedules and routines, record keeping, documentation, etc.
- Perform skills training as identified in each participant’s Individual Service Plans which include but are not limited to the following: community inclusion, social skill building, skill acquisition, and behavioral support needs.
- Work with individuals with histories of aggression.
- Participate in the assessment process and collection of data; Report writing.
- Attend required training and staff meetings.
- Other duties as assigned.

Engaging Support Plans
Services should be rooted in Person Centered Thinking. The goal of this program is to increase the quality of our participant’s lives. This is achieved by helping our participants create goals and plan activities that are meaningful to them.
Reasearched Based Strategies
Evidence Based Practices (EBP) will be researched when designing treatment plans to help ensure quality interventions are being utilized to teach skill acquisition and address behavior challenges. These include EBPs found within the model of Applied Behavior Analysis, including but not limited to Discrete Trials Training, Pivotal Response Training, or Picture Exchange Communication.
Effective Approach
Data is useful in recording a participant’s progress in comparison to the initial baseline of the skill acquisition target or behavioral goal being tracked. Measuring intervention effectiveness will help direct the course of intervention to ensure long lasting, positive changes in the life of the participant.
