Community Liaison - Home Health

About the Role
Corus Home Health is seeking a driven and experienced Community Liaison to build strategic referral relationships and expand awareness of our services in the Albuquerque area. Based fully in Albuquerque, this role plays a vital part in connecting patients to the high-quality, compassionate care we provide in the home.

Key Responsibilities

• Promote the full spectrum of Corus Home Health services to physicians, hospitals, skilled nursing facilities, and other referral sources
   
• Educate referral partners and the community about home health benefits, eligibility, and value
   
• Coordinate referrals and ensure timely follow-up with patients, families, and providers
   
• Support marketing and outreach efforts through professional presentations and events
   
• Analyze referral patterns and identify new opportunities for outreach
   
• Collaborate closely with clinical teams to ensure smooth handoffs and exceptional service
   
• Maintain accurate tracking of outreach efforts, relationships, and outcomes
   
• Uphold all regulatory guidelines, including HIPAA compliance and ethical marketing standards

What We’re Looking For

• Proven experience in healthcare marketing, referral development, or physician outreach
   
• Strong knowledge of the home health industry and Medicare benefit structure
   
• Professional communication skills, with the ability to engage physicians, discharge planners, and community partners
   
• Goal-oriented mindset with the ability to self-manage and follow through
   
• Commitment to integrity, relationship-building, and patient-first advocacy
   
• Valid driver’s license and reliable transportation
   
• Bachelor's degree preferred
